We kindly request at least 3 weeks' notice in advance of your deadline to ensure adequate time for review and preparation. Final timeline is subject to cooperation with PHC staff for gathering information. We will require PRM assistance in gathering content and reaching out to the appropriate PHC staff.

Once the form is received, a member of the DR team will confirm your submission and kick off the report process.
